A. Inspect at a minimum daily, mark a “+” for items being performed well or in good condition Mon Wed Tues Thurs Fri Sat Sun
B. For items that need improvement or corrective action, mark a “∆” and comment on the back any action needed/taken

1. Trench Protection


Delineation, barricades and signage in place, trench plates in place and not displaced
Benching or sloping done when appropriate (as determined by competent person, but required 5 feet or more in depth)
Trench shield (box), trench plates and/or shoring in place, in good condition and secure
Rebar caps or other protection on all impalement hazards
Fall protection provided for trenches greater than 6’ in depth

2. General Site Conditions


Surface encumbrances or impediments removed or supported
Spoils and other materials are set back at least 2’ from edge of excavation
Sources of vibration (equipment, traffic etc.) are not too close to excavation and workers
Employees not working under suspended loads, around equipment, around vehicles being loaded or in actively excavated holes
Employees are wearing garments of high visibility reflective material when working around moving equipmen

3. Access/Egress


Ladders provided for all trenches greater than 4’ in depth, proper ladder used and ladder secured and extends 36” out of hole.
Walkways and bridges over excavations (required for trenches >6’) are necessary width, have toe boards, guardrails etc. in place
Unobstructed lateral access to a means of egress no less than 25’ of travel distance

4. Utilities


All utilities have been precisely marked, potholed and/or protected
Sewer and gas lines are exposed, supported and/or protected
Critical and hazardous utilities are potholed at a minimum of every 25’ horizontally – pot holes dug (air, water or vacuum systems preferred) in 24” radius around utilities. No mechanical equipment should be used for pot holing

5. Trench Conditions


Employees protected from water accumulation and/or surface runoff
Trench inspected for potential cave ins, sloughing, cracking etc

6. Hazardous Atmospheres


Atmosphere tested where oxygen deficiency, explosion, combustion or other harmful contaminants possible
Adequate precautions taken to protect atmosphere, emergency equipment available and employees trained
